## What to try while the joint is still moving

Soreness that eases with gentle motion often allows some daily activity. Short walks and easy movement can keep stiffness from taking over. Stop if the leg feels unsteady or the ache turns sharp. A task isn't worth a fall or a worse flare.

Try warmth before an ordinary task and cold afterward for swelling. A properly fitted cane or brace may make movement steadier. Your own doctor or nurse can review medicine that may be safe. Tell them if the joint is swollen, warm, or suddenly worse.

## When to arrange a regular visit

Arrange a visit if repeated soreness begins to alter your walk. Trouble with sleep, dressing, or reaching also deserves attention. The exam can tell joint soreness from an ache in a nearby tendon. It may also show whether past damage still affects movement.

Bring your health history, medicine names, and earlier X-rays to the visit. Tell the provider what home care you tried and what happened. Request a plain account of what the exam showed. Then discuss choices that fit the cause of the soreness.

## When to seek care without delay

Fever with a hot, badly swollen joint needs medical help quickly. A new injury also needs care if you cannot bear weight. Spreading redness or drainage after treatment may mean infection, especially with fever. Sudden numbness or weakness calls for quick attention too.

These problems need an urgent exam before any planned joint treatment. Call your own doctor or seek nearby urgent care when they occur. Severe symptoms may require emergency care, especially after a fall. Don’t drive past closer help to reach an elective clinic visit.

## What to ask about price and coverage

Ask for the full price before agreeing to a procedure. The written amount should state what the visit and follow-up include. Medicare has limited PRP coverage, and private plans set their own rules. A clinic estimate isn’t the same as a coverage decision.

Call the number on your insurance card before paying. Ask whether approval is required and whether any part is excluded. Keep the written reply with the clinic estimate. Cost matters because the result for any one joint remains uncertain.

The Cochrane review of exercise for knee osteoarthritis found low- to moderate-certainty evidence that exercise probably improves pain, physical function and quality of life in the short term - but when the results were compared against established minimal important difference thresholds (12 points for pain, 13 for function, 15 for quality of life on 0-100 scales), the confidence intervals either did not reach those thresholds or spanned both important and unimportant improvement. A systematic review and meta-analysis of adverse events in randomized trials of intra-articular PRP for knee osteoarthritis found PRP associated with mild, transient adverse events - knee pain and swelling - that typically resolve without intervention, and more frequent with LEUKOCYTE-RICH formulations. Leukocyte-poor PRP had a safety profile similar to hyaluronic acid. Knee stiffness was more frequent with PRP than with normal saline (P=.031). No severe adverse events were reported in any group.

Medicare's national coverage policy covers autologous platelet-rich plasma ONLY for patients with chronic non-healing diabetic, pressure and/or venous wounds, and only within an approved coverage-with-evidence-development clinical study. There is no Medicare national coverage for PRP in osteoarthritis or tendinopathy, which is why these injections are billed to the patient as cash-pay.

5. The devices used to spin PRP at the point of care are cleared by FDA as clinical centrifuges, product code JQC, a Class I device under 21 CFR 862.2050 - for example the Biomet GPS Platelet Separation Kit (K030555, cleared 2003) and the Harvest SmartPrep2 / SmartPrep Platelet Concentration System (K103340, cleared 2010). That clearance covers the equipment that separates blood. It is not an FDA approval of platelet-rich plasma as a treatment for osteoarthritis, tendinopathy or any other orthopedic condition, and copy must never blur the two.

The American Academy of Physical Medicine and Rehabilitation convened a technical expert panel that ran a structured literature review (2023, updated through June 2025) and a modified Delphi process, and issued five evidence-based clinical recommendations plus 11 consensus-based best practices for PRP in knee osteoarthritis. The statement is explicit that orthobiologic therapies 'remain an evolving area of practice' and that robust, dose-dependent randomized controlled trials are still needed to establish PRP's clinical effects.
